Leadership qualification process
GO does not inform County Coordinators of proposed new appointments or changes of a role requiring additional LQ modules. Please complete the form below and details will be sent to the County Leadership Coordinator. She will send a leadership pack to the mentor. If the new Leader is not known to the mentor then the Commissioner should pass on the relevant details to the mentor.
Before completing this form please ensure that you have the applicant's membership number from GO and that the applicant's status on GO is "Active" (ie for members over 18 years of age a DBS check has been completed and two satisfactory references obtained). The status of "Active Pending" means that one or more of those checks are still outstanding and must be obtained before formal training can commence.
